Immunization Requirements and Schedule

Requirements: Effective August 2008

For students entering 7th grade will be required to have received all of the following:

  • 5 DTaP - 4 if the 4th dose was given on/after the 4th birthday
  • 4 Polio - 3 if the 3rd dose was given on/after the 4th birthday
  • 2 Measles
  • 2 Mumps
  • 2 Rubella
  • 3 Hepatitis B
  • 2 Varicella - waived if the parent or guardian presents a Department-supplied form indication the student has a history of the disease
  • 1 Meningococcal conjugate vaccine (only required for students attending residential-based schools such as boarding schools)
  • 1 Tdap booster

Students entering a post-secondary school will be required to have received the following:

  • 1 Tdap/Td booster administered within the past 10 years
  • 2 Measles
  • 2 Mumps
  • 2 Rubella
  • 3 Hepatitis B
  • 1 Meningococcal conjugate vaccine (only required for first year students living in dormitories)
  • 2 Varicella - waived if student presents a Department-supplied form indication the student has a history of the disease

Vermont Recommended Immunization Schedule:

Birth Hepatitis B        
2 months Hepatitis B + DTaP + Polio HIB PCV Rotavirus  
4 months Hepatitis B + DTaP + Polio HIB PCV Rotavirus  
6 months Hepatitis B + DTaP + Polio HIB PCV Rotavirus Influenza (every flu season until age 5)
12-15 months MMR + Varicella HIB PCV Hepatitis A  
15-18 months DTaP     Hepatitis A (6 months after 1st dose)  
4-6 years MMR + Varicella DTaP Polio    
11-12 years Tdap (Td booster every 10 years after) Meningococcal (1 dose) HPV (3 dose series for females) Varicella? (no vaccine or disease? 2 doses needed) Influenza? (For individuals at higher risk for complications)
13-18 years Access and catch up for any vaccines        

Name/key of immunizations:

DTaP: diphtheria, tetanus (lock jaw), pertussis (whooping cough)

Hepatitis A & B: serious liver diseases

HPV: human papillomavirus, causes cervical cancer

Hib: haemophilus Influenzae, a brain, throat and blood infection

Influenza: a very contagious  viral infection

Meningococcal: a blood infection and/or meningitis

MMR: measles, mumps and rubella

PCV: pneumococcal, a blood, lung, and brain infection

Polio: causes paralysis

Rotavirus: causes severe diarrhea

Tdap: tetanus, diphtheria and pertussis for children 10 or younger

Varicella: Chickenpox
